Client Evaluations
Client evaluations are similar to Client Assessments but instead of numeric answers you can choose between several different question types:

The Evaluation forms are designed in the Administration Module and responses are filled in on the client record on the Client Evaluations tab.
Creating an Evaluation
Pick the Client Evaluations option from the main Administration Menu. Any existing forms will be shown in the list and you can create a new form with the New Client Evaluation Form button. Initially all you have to specify is the name of the form.
After saving the from you can click the Form Name in the list and the click Add Question to add a question and repeat until you have all the questions required.
Tip
There is a Preview Form button to show you what the form will look like when used in the client pages.
Warning
Once there are responses on file for the form it is not possible to modify the questions so be sure that you have finished editing before adding the form to a client. You can still add new questions though.
Filling in an Evaluation
Bring up the required client in the Client Management module and the click on the Client Evaluations link at the top of the page.
The Client Evaluations page has three parts. At the top is a dropdown list of the available evaluation forms that have been created. To enter responses simply pick the required form and click Add to client.
The evaluation form will then appear in the Evaluations given section of the page. Just click on the form in this list and you will see a set of question on the form ready to fill in.
Evaluation Analysis
Once responses have been entered then you can use the Analysis feature which is at the bottom of the page. Pick the form for which you want the analysis from the drop down list and then click Analyse.
The analysis will show each of the questions along with the responses. If the form has been answered several times then both sets of responses will be shown allowing you to judge whether there has been any improvement. The analysis can be exported to a csv file by clicking the button at the bottom of the page.
